If you work with Power Automate, you certainly will discover the limits. One of these difficult limits, is the 30-day limit of a workflow. If the workflow is not completed within these 30 days, a time-out occurs. Certainly for workflows with multiple approvals, this can happen.

But if nothing is done, the workflow goes into time-out without notifying a user. So the user thinks everything is ok, until he checks the status.
